Level 4 Home Care Packages
This level is suited for older Australians with complex needs, including service details, hours, and funding options to enhance care.
A Level 4 Home Care Package offers high-level Home Care to you or your loved ones. It is the top-level government-subsidised home support available for older Australians. It is designed for people whose health is declining and are struggling to stay independent at home on their own.

A Level 4 Aged Care Package was the highest level of Home Care for individuals with complex needs (see the eight new funding levels through Support at Home), such as nursing requirements. Depending on the type of care your loved one needs, this level can assist in various tasks such as personal care, nursing, and cleaning.
A Level 4 Aged Care Package offers a flexible number of care hours, tailored to each individual’s needs. Typically, a level 4 package offers between 18.5 to 25 hours of support every fortnight, which can vary depending on your provider’s fees. This approach ensures that each person receives comprehensive and supportive care specific to their requirements.
This Level 4 Aged Care Package is now classified as a Transitioned HCP Level 4 under the new aged care program, Support at Home. To see what’s changed since Support at Home (effective 1 November 2025), please see our all-inclusive guide here.
As the recipient of a My Aged Care Level 4 Home Care Package, you also get to choose your approved Home Care Package provider.
The annual government subsidy for a Level 4 Home Care Package is $63,758.20 a year. It’s important to note, that while the former Home Care Packages’ program comprised a basic daily fee and a fortnightly fee, the new Transitioned HCP Level 4 under Support at Home is no longer structured this way.
If you previously contributed fees under the Income Tested Fee (ITF) arrangement and are a Grandfathered client, which means, you either held or were approved for a Home Care Package (HCP) on or before 12 September 2024 – you were automatically transitioned to the Support at Home program under the “no worse off” policy.
Ultimately, this means that your co-contributions under the new Support at Home program will be equal or less than what you paid before. Here, it’s also worth knowing that the basic daily fee no longer applies under Support at Home.

You can apply for aged care funding via the My Aged Care (MAC) portal or simply call MAC at 1800 200 422. Once you have applied, My Aged Care will arrange for an assessor to visit you in your home and determine the level of care you need.
You may be eligible for Support at Home funding if you are aged 65 years of age or above, are an Australian resident, and require assistance with tasks at home. Aboriginal or Torres Strait Islander individuals are eligible for a Home Care Package from 50 years old.
